| Patients are seen by appointment Monday through Friday from 08:30 till 5:00. Our staff will advise you of the days, times and physicians that are available at the various locations. If you are unable to keep a scheduled appointment, or are going to be late, please call our office as soon as possible. Every effort is taken to try to see our patients on time, however due to the nature of a Cardiology practice there will be emergencies. |

| Emergency Care |
|
| Our physicians are available 24 hours a day. If you
have any emergency please call the office that serves you. In a life
threatening EMERGENCY call 911 or go to the Emergency room and ask
that your physician be called. |

|
|
| Medication Refills |
|
| If you need a refill on your medications, please call
your pharmacy one week in advance. They will check with your
physician regarding your refill requirements. We do not refill
medications after hours or on weekends. Please plan ahead by calling
your pharmacy before you run out of medications. |

|
| Telephone Calls |
|
| When calling our office for advice regarding a
medical condition, please review your medications, including
doses, prior to placing your call. This will help us assist you.
The clinic staff will give the information to your physician and return your call
as soon as possible. Our nurses and staff are specially trained to answer your questions
and pass information between you and your physician. |

| Insurance and Business Office |
|
| Heart & Vascular Clinic is contracted with most insurance plans. Please be prepared to provide your
insurance information at the time of your visit. Your insurance benefits will then be verified by our staff. If
you do not have insurance coverage, payment will be expected at the time of service. If your
plan requires a co-pay, it will be due at time of visit. Should this present a hardship on you, our
business office staff will be happy to make payment arrangements. Payment may be made by cash, check
or MasterCard, Visa and Discover credit cards. |
